Mite Jamboree Sign Up Form

🏒 How to Sign Up?

To sign your team up, please fill out the form provided on this page. We apologize in advance if form is confusing. Our sign up system is meant for player camp sign up so please follow the format below as an example.
 
After you submit your form. You will receive a confirmation/invoice email to the emails provided on your form automatically from the system within 1 – 2 minutes of your submission. Please check for this message and that your sign up is correct. Messages do sometimes go to SPAM/JUNK Folders so make sure to check their if not received. STEP #1 = FILL OUT COACH & MANAGER CONTACT INFORMATION

PARENT FIRST NAME (TEAM MANAGER) = Nick Weigel

PARENT LAST NAME (COACH NAME) = Mark Zarbo

EMAIL (MANAGER EMAIL) = Manager@yahoo.com

SECONDARY EMAIL (COACH EMAIL) = Coach@yahoo.com

STEP #2 = HOW MANY TEAMS ARE YOU SIGNING UP?

Under the contact information section it asks “How many players are you signing up?” Use this question as “How many teams are you signing up?” If signing up 1 team, select 1. This will generate 1 form to fill out for that team. If signing up 2 or more teams select the appropriate number. This will generate 2 or more forms. Please note you must fill out a form for each team!
 

STEP #3 = FILL OUT TEAM INFORMATION

PLAYER FIRST NAME (TEAM NAME) = Caz 7u

PLAYER LAST NAME (TEAM NAME AGAIN, SORRY!) = Caz 7u

PLAYER BIRTH YEAR = 2019

PLAYER CURRENT LEVEL = Select level from drop down list

PLAYER CURRENT TEAM (TEAM NAME AGAIN, LAST TIME PROMISE!) = Caz 7u
 

STEP #4 = SELECT THE JAMBOREE SIGNING UP FOR

Under the season program list, check mark the box next to the jamboree you are looking to sign your team up for. After check marking that box you must then check mark the date, time under that specific jamborree tab. If interested a us adding a Jamborree in March select that tab and then check mark the box next too “Yes, I am interested in receiving information for a Jamboree in March.”
